Your ETP registration submission was approved by a C4MH physician:

The next step is your Lab tests at Quest labs.

Prepayment for lab service and initial exam is required:

1. Click on the C4MH location you prefer (below).
2. Follow instructions for pre-payment of exam and lab (total cost $45).
3. Once C4MH receives payment we will send you an e-mail with Quest Lab Orders along with complete instructions.
(The C4MH ETP response to payment is NOT automated and may take up to 24 hours to be issued)

Please note that physician's initial exams are performed at one of the following five locations: Brandon, Pinellas Park, Sarasota, Venice and Ft. Myers.

After initial exam patients may go to any location for treatments.
